Please complete a Request for Certification of Information form prior to requesting the notarization or apostilling of a transcript or diploma. This form may be accessed by logging into your myLSUS account and clicking on the Records/Forms tab at the top of the page. When completing the form, please follow the steps listed below:

Once we receive the apostilled documents back from the Louisiana Secretary of State’s Office, we will send them to you at the address provided along with a tracking number.
A fee of $20 will be assessed to your student account for each document processed. You may pay the fee(s) via your myLSUS account. Additional fees for postage will be assessed for documents sent back to students at an address outside the continental United States.
Note: LSU Shreveport only offers document authentication at the state level (notary and apostille). We do not facilitate any additional levels of authentication.
During the 1993 Regular Legislative Session, the Legislature passed House Bill 535, and the Governor later signed into law, Act 253 of 1993 which enacted Louisiana Revised Statutes (L.R.S.) 17:3129.3. This law created the Louisiana Classroom Teacher Enrollment Program (CTEP), which, if several stipulations are met, allows classroom teachers to attend Louisiana’s public colleges and universities tuition-free (all other fees must be paid by the teacher) on a space available basis.

Graduates are strongly encouraged to order prior to the deadline mentioned above. Graduates must be in full regalia for their respective degrees to participate in commencement ceremonies.”
