Notarize and Apostille LSUS Transcripts and Diplomas

Please complete a Request for Certification of Information form prior to requesting the notarization or apostilling of a transcript or diploma. This form may be accessed by logging into your myLSUS account and clicking on the Records/Forms tab at the top of the page. When completing the form, please follow the steps listed below:

1. Complete the top portion of the form. 
2. Select “Check here if information requested is for all prior terms of enrollment at LSUS”.  
3. Choose the option “Other” on the form, and answer the following questions in the space provided:
   1. What documents needs to be apostilled?
   2. What country will it be used in?
   3. For what purpose are you needing the documents?
   4. Indicate that LSUS needs to notarize the documents and the Louisiana Secretary of State’s office needs to apostille the documents.
   5. Please note that apostilled documents can only be used for purposes outside of the United States per the Louisiana Secretary of State’s website. 
4. In the address section, input the address where you would like the apostilled documents be sent.

To request your official LSUS transcript be (notarized &) apostilled by the Louisiana Secretary of State’s Office, please follow these steps: 

• Request a paper copy of your official LSUS transcript via Parchment (See instructions listed above under the heading “Transcript Request.”). Have your transcript sent to LSU Shreveport, One University Place, Office of Admissions and Records,  AD 168, Shreveport, LA 71115.
• Notify the Records Office at registrar@lsus.edu that you’ve made the request. Make sure to provide us with a current address. If you live outside of the continental United States, please make sure to correctly format your address to prevent possible delays.

To request your diploma be (notarized &) apostilled by the Louisiana Secretary of State’s Office, please follow these steps: 

• Request a copy of your diploma using the Duplicate Diploma form found under Records/Forms on myLSUS. Have your diploma sent to LSU Shreveport, One University Place, Office of Admissions and Records, AD 168, Shreveport, LA 71115. If you live outside of the continental United States, please make sure to correctly format your address to prevent possible delays.
• Notify the Records Office at registrar@lsus.edu that you’ve made the request.

Once we receive the apostilled documents back from the Louisiana Secretary of State’s Office, we will send them to you at the address provided along with a tracking number.

A fee of $20 will be assessed to your student account for each document processed. You may pay the fee(s) via your myLSUS account. Additional fees for postage will be assessed for documents sent back to students at an address outside the continental United States.

Note: LSU Shreveport only offers document authentication at the state level (notary and apostille). We do not facilitate any additional levels of authentication.

Louisiana Classroom Teacher Enrollment Program (CTEP)  

During the 1993 Regular Legislative Session, the Legislature passed House Bill 535, and the Governor later signed into law, Act 253 of 1993 which enacted Louisiana Revised Statutes (L.R.S.) 17:3129.3. This law created the Louisiana Classroom Teacher Enrollment Program (CTEP), which, if several stipulations are met, allows classroom teachers to attend Louisiana’s public colleges and universities tuition-free (all other fees must be paid by the teacher) on a space available basis.

Policy Restrictions: 

• You must apply and be admitted into the University – either as degree seeking or non-matriculating.
• Do not add the course(s) to your schedule yourself. CTEP operates on a space available basis. Once the last day to add has passed, eligible students will be added to the course if there are seats available. Adding the course(s) on your own will result in you not being able to use CTEP funds.
• The course should lead to an education degree/certification area or increase the content knowledge of the subject area currently being taught by the applicant.
• Course enrollment is limited to 6 semester hours per traditional 15-week semester (spring, fall)
• CTEP applications must be submitted before the last day to add courses for the term in which you’re intending to take the course(s).

Graduation Information

Spring deadline to order graduation regalia: April 4th, 2025

NOTE: While regalia is available for purchase on campus and at the ceremony on the day of, please note that supplies are extremely limited. Graduates are strongly encouraged to order prior to the deadline mentioned above. Graduates must be in full regalia for their respective degrees to participate in commencement ceremonies.”
